Responsibilities include client assessments, developing and coordinating care plans, conducting home and office visits, providing health education, and mentoring staff and students.
Qualifications include a Nursing Bachelor’s Degree or equivalent experience, RN licensure, BLS certification within 90 days, bilingual in English/Spanish, and relevant clinical experience.
