Psychiatric Mental Health Nurse Practitioner (PMHNP) – Rhode Island – Optum

Role Overview:
Provide consultative behavioral health services to patients in long-term and community care settings as part of a team dedicated to holistic, patient-centered care. This autonomous role involves managing behavioral conditions, conducting assessments, documenting findings per regulations, and collaborating with care teams to develop and revise personalized care plans. The position emphasizes education, care coordination during transitions, and implementing non-pharmacological approaches aligned with patient goals. The role offers a flexible schedule with no weekends or major holidays, focusing on improving patient outcomes and reducing unnecessary hospitalizations.

Qualifications & Skills:

  • Unrestricted NP license and PMHNP certification
  • DEA licensure or ability to obtain
  • 1+ year experience in long-term or community care settings (preferred 3+ years)
  • Reliable transportation and valid driver’s license for travel
  • Ability to assess, plan, coordinate, and evaluate behavioral health needs with a multidisciplinary team
  • Strong communication and creative problem-solving skills
  • Key Responsibilities:

  • Manage behavioral health conditions, perform assessments, and document findings
  • Develop, implement, and revise care plans in collaboration with the care team
  • Coordinate care transitions and evaluate plan effectiveness and cost-efficiency
  • Educate staff and families on behavioral health approaches and patient needs
