We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients’ homes, supervised by a Registered Nurse.
Responsibilities include:
- Delivering and coordinating quality patient care according to physician orders
- Assessing patient condition, monitoring changes, and reporting as needed
- Documenting assessments, nursing actions, and care activities in a timely manner
- Implementing and updating care plans, recognizing patient needs, and preventing risks
- Educating patients and families on disease management, self-care, and prevention
- Ensuring infection control and maintaining confidentiality
Qualifications and skills:
- Current, active LPN/LVN license in the practice state
- Current CPR certification
- TB test within the last 12 months
- Preferred: six months of recent clinical experience or home health background
- Strong attention to detail, organization, communication, and problem-solving skills
- Physical ability to lift 50 pounds, reposition patients, and perform home care tasks
- Reliable transportation and ability to travel
