Role: Private Duty LPN
Responsibilities:
Provide one-on-one skilled nursing care in patients’ homes Administer medications, perform wound care, and monitor chronic conditions Support daily living activities and assist with personal needs Collaborate with a multidisciplinary team and families to implement individualized care plans
Qualifications & Skills: Licensed Practical Nurse (LPN) license Compassionate, patient-focused attitude Strong communication and teamwork skills Ability to follow detailed care plans and adapt to patient needs Experience in home healthcare preferred
Key Duties: Deliver personalized nursing care in a home setting Ensure patient comfort and safety Maintain accurate documentation and adhere to healthcare standards
