Private Duty Nurse (LPN)
Provide compassionate, one-on-one skilled nursing care to patients in their homes, supporting both pediatric and adult care needs.
Key Responsibilities
- Perform patient assessments and follow individualized care plans
- Administer medications and monitor patient condition
- Manage trach/vent support and G-tube care
- Provide wound care and other skilled nursing interventions
- Accurately document care and maintain required clinical records
- Collaborate with families and the clinical team to ensure consistent, patient-first care
Qualifications & Skills
- Current LPN licensure
- Strong clinical skills in medication administration and specialized care (trach/vent, G-tube)
- Ability to perform assessments and deliver high-quality wound care
- Excellent communication, documentation, and teamwork skills
- Compassionate, patient-centered approach to home care
