Responsibilities include:
Providing comprehensive adult primary care, including diagnosis and personalized treatment plans
Delivering preventive, wellness, and chronic care services
Using data to improve outcomes through value-based care
Collaborating with interdisciplinary teams for care coordination
Maintaining accurate EHR documentation and driving practice innovation
Qualifications:
MD or DO with active US medical license
Strong adult primary and chronic disease management skills
Experience in Family Medicine, Internal Medicine, or related fields; interest in value-based care preferred
Excellent communication skills
