As the Skilled Nursing Facility physician, you will oversee medical care for Participants in SNFs, manage acute and chronic conditions, and collaborate with interdisciplinary teams to ensure seamless care transitions.
Responsibilities include rounding on SNF Residents, conducting enrollment assessments, developing care plans, and facilitating discharge back to the community. On-call coverage is shared within a team.
Ideal candidates are mission-driven, collaborative, and experienced in geriatrics or skilled nursing care. Requirements include an active PA medical license, board certification in Internal Medicine or Family Medicine, and strong communication skills.
