Seeking a Primary Care Physician for a senior-focused program serving individuals 55+ with chronic illnesses or disabilities.
Responsibilities include delivering comprehensive primary care, developing individualized care plans, managing chronic and acute conditions, providing preventive services, and coordinating with an interdisciplinary team.
Qualifications: MD/DO with interest in geriatrics, Board certified or eligible in Internal Medicine, Family Practice, or Geriatric Medicine, licensed and DEA registered, with at least one year of experience working with the elderly.
