Role: Primary Care Physician responsible for providing equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.
Responsibilities:
- Assess, diagnose, and oversee primary care for patients
- Prescribe medications and manage treatment plans
- Maintain electronic medical records using Canopy and Greenway
- Collaborate with regional and center leadership to achieve health quality goals
- Work with practice managers to direct care teams
- Perform additional duties as assigned
Qualifications and Skills:
- MD or DO with internal medicine or family medicine certification (or eligibility)
- Active, unrestricted state license and DEA license
- US work authorization
- Preferred: Geriatrics fellowship, experience with older adults, value-based care, outpatient primary care, bilingual in Spanish
- Motivated to deliver high-quality care, including new graduates
