Role Summary
Seeking a Nurse Practitioner or Physician Assistant to deliver patient-centered primary care within a collaborative, physician-led team.
Key Responsibilities
- Conduct comprehensive patient assessments
- Manage acute and chronic conditions
- Order, interpret, and act on diagnostic tests
- Prescribe medications and develop treatment plans
- Coordinate care with specialists, behavioral health, and ancillary services
- Provide preventive care, health education, and ongoing patient/family communication
- Maintain strong patient relationships to support seamless, coordinated care

- Clinical expertise in primary care and patient assessment
- Ability to diagnose, treat, and manage multiple conditions
- Proficiency in diagnostic evaluation and medication management
