Role Overview:
Provides direct outpatient patient care in collaboration with healthcare providers and the care team, developing and implementing individualized care plans to ensure high-quality, cost-effective outcomes. Supports clinical staff and assists providers with daily patient management, documentation, and patient education.
Key Responsibilities:
- Collaborate with PCPs and specialists to develop, review, and update care plans.
- Assess and evaluate patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Ensure adherence to clinical protocols, patient assessment guidelines, and compliance standards.
- Support documentation of assessments and treatment protocols.
- Educate patients and families, providing verbal and written instructions, and assess their ability to receive education.
- Maintain safety standards, working in a constant state of alertness.
Qualifications & Skills:
- Nursing Diploma or Associate’s degree in Nursing required; Bachelor’s preferred.
- 2-5 years of nursing experience; supervisory experience is optional.
- Active RN license and Basic Life Support (BLS) certification required.
- Experience in GI/endoscopy and scheduling is preferred.
- Strong critical thinking, communication, and patient education skills.
