Postpartum – Clinical Nurse – Live Well Homecare

Role Overview: Provides direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards. Focuses on promoting patient and family-centered care, educating on illness management, and ensuring safety, quality, and regulatory compliance. Develops and documents individualized care plans in collaboration with the healthcare team, addressing physical, psychosocial, and emotional needs while respecting patient preferences and cultural values. Maintains a safe work environment by working in a constant state of alertness.

Qualifications & Skills:

Nursing Diploma/Associate’s Degree in Nursing (required); Bachelor’s Degree preferred.
RN license required.
No prior experience necessary; two years of experience preferred.
Strong critical thinking, communication, and patient education skills.
Ability to work safely and effectively in a healthcare setting.
