Post-Acute and Long-Term Care (PALTC) – Wound Care Advanced Practitioner (WCAP)

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), specializing in wound assessment, treatment, and management within a PALTC facility.

  • Perform comprehensive evaluations of residents at risk for or with existing wounds.
  • Develop and update individualized wound care plans, including advanced treatments like debridement, dressings, and negative pressure therapy.
  • Collaborate with interdisciplinary teams to ensure integrated wound prevention and care strategies.
  • Implement evidence-based prevention protocols and provide staff education.
  • Maintain detailed documentation and participate in quality improvement activities.
  • Serve as a resource for staff, residents, and families, addressing wound care education, infection management, and regulatory compliance.
  • Utilize technology such as EHR systems and telemedicine tools.

  • Qualifications include a relevant degree (BS-PAS/MPAS/DMSc/MSN-NP/DNP), minimum 2 years of experience (long-term care preferred), and board certification (AANP/ANCC/PA-C). Specialty wound care certification is highly preferred.
