Post-Acute and Long-Term Care (PALTC) – Wound Care Advanced Practitioner (WCAP)

The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, such as a Nurse Practitioner (NP) or Physician Assistant (PA), with specialized wound management training.
They assess, treat, and manage wounds in residents of long-term care facilities to reduce complications, improve outcomes, enhance patient and family experience, prevent hospitalizations, and lower costs.
Key duties include comprehensive assessments, individualized care planning, advanced wound treatments, collaboration with healthcare teams, prevention strategy development, staff education, documentation, quality improvement, infection management, regulatory compliance, patient advocacy, and resident/family education.
The role requires excellent communication, leadership, technology proficiency, physical mobility, and a minimum of 2 years experience, with long-term care experience preferred. Certification in wound care and relevant licensure (APRNP/PA) are essential.
