Post-Acute and Long-Term Care (PALTC) – Wound Care Advanced Practitioner (WCAP)

The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), with specialized wound management training.

They are responsible for

Assessing and managing wounds in long-term care residents, including pressure injuries, ulcers, and surgical wounds.

Developing individualized care plans and performing advanced wound treatments such as debridement and negative pressure therapy.

Collaborating with interdisciplinary teams and educating staff on wound prevention and care best practices.

Monitoring outcomes, ensuring regulatory compliance, and serving as a resource for residents, families, and staff.

Addressing infections promptly and advocating for residents’ dignity and autonomy.

Qualifications include a relevant degree, 2+ years of experience, board certification (AANP, ANCC, PA-C), and preferred specialty certification in wound care.
