Role Overview: Provide comprehensive outpatient primary care services to diverse patient populations, including managing chronic conditions, preventive care, and routine screenings. Collaborate with healthcare teams, maintain accurate electronic health records, and participate in quality improvement initiatives. Engage in community outreach and stay current with medical best practices.
Responsibilities include:
- Delivering patient-centered primary care across all ages
- Developing personalized treatment plans and managing chronic illnesses
- Performing wellness visits and screenings
- Collaborating with multidisciplinary teams to optimize patient outcomes
- Maintaining thorough electronic health records
- Participating in quality improvement and community health initiatives
Qualifications & Skills:
- Board Certified or Eligible in Family Medicine, Internal Medicine, or Med Peds
- Strong clinical and diagnostic abilities
- Excellent communication and teamwork skills
- Commitment to patient-focused care and relationship building
- Willingness to work flexible hours, including some early mornings, evenings, and weekends
- Experience in outpatient primary care is preferred but not required for new graduates
