Must be board-certified with a medical degree, completed a 3-year primary care residency, and hold a Missouri medical license. Provides patient-centered care within a team-based model, coordinating services, ensuring patient rights, and attending trainings and meetings. Supports continuity of care across various sites and assists in equipment and supply needs.
