Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.
Join an interdisciplinary team to develop and monitor personalized care plans, providing geriatric and medical expertise.Supervise nurse practitioners and clinical staff, offering preventive, episodic, and chronic care, including physical and mental health assessments.Evaluate members, interpret diagnostics, manage medications, and coordinate referrals, ensuring tailored, comprehensive care.Educate patients and families on health maintenance and illness management.Travel locally as needed, with a schedule Monday to Friday, 8:00AM–4:30PM, with remote after-hours on-call rotation.Qualifications include a Doctorate in Medicine, licensure, DEA registration, Board Certification or eligibility in Family or Internal Medicine, and recent primary care experience with at-risk or elderly populations.Benefits include competitive pay, flexible schedules, paid time off, health/dental/vision coverage, retirement plans, and employee programs.
