Physician

Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.

  • Participate in developing and monitoring personalized care plans within an interdisciplinary team.
  • Provide preventive, episodic, and acute care, including physical, mental, and functional assessments.
  • Supervise nurse practitioners or clinical nurse specialists at designated sites.
  • Manage chronic and acute conditions, adjust medications, and interpret diagnostic tests.
  • Educate patients and families on health maintenance and illness management.
  • Travel locally using personal vehicle; work hours are Monday to Friday, 8:00AM–4:30PM, with after-hours on-call rotations.

  • Requirements: Doctorate in Medicine, MI license, DEA registration, board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, with at least 1 year primary care experience with elderly or at-risk populations.
