Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and disabled individuals in maintaining independence.
Participate in developing and monitoring personalized care plans within an interdisciplinary team. Provide preventive, episodic, and acute care, including physical, mental, and functional assessments. Supervise nurse practitioners or clinical nurse specialists at designated sites. Manage chronic and acute conditions, adjust medications, and interpret diagnostic tests. Educate patients and families on health maintenance and illness management. Travel locally using personal vehicle; work hours are Monday to Friday, 8:00AM–4:30PM, with after-hours on-call rotations. Requirements: Doctorate in Medicine, MI license, DEA registration, board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, with at least 1 year primary care experience with elderly or at-risk populations.
