Seeking a dedicated Physician to join a community-focused, mission-driven organization supporting elderly and adults with disabilities.
Full-time role based in Mt. Pleasant or Houghton Lake, collaborating with an interdisciplinary team to develop and monitor personalized care plans.Provide preventive, episodic, and chronic care, including physical, functional, and mental health assessments, and manage acute and chronic conditions.Supervise nurse practitioners or clinical nurse specialists, offer health education, and facilitate referrals.Requires MD/DO, board certification or eligibility in Family Medicine, Internal Medicine, Geriatrics, or Hospice, with at least 1 year primary care experience with at-risk or elderly populations.Schedule: Monday to Friday, 8:00AM–4:30PM, with after-hours on-call rotation.Benefits include competitive pay, generous PTO, health/dental/vision insurance, retirement plan, flexible benefits, and more.
