We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities.
Responsibilities include conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy to improve mobility, independence, and quality of life, and documenting care within our EMR system.
They will collaborate with an interdisciplinary team of healthcare professionals, educate patients and caregivers on safety and aging-in-place strategies, and incorporate telehealth when appropriate to enhance access and flexibility.
Qualifications include a valid PT license, basic computer skills, CPR certification, and physical ability to perform in-home care tasks such as lifting, repositioning, and assisting with transfers.
