Role: Physical Therapist (PT)
Responsibilities:
- Plan, organize, develop, and implement Physical Therapy services in compliance with applicable standards and regulations.
- Assist residents in achieving and maintaining optimal physical, mental, and social well-being.
- Participate in discharge planning, develop care plans, and conduct resident assessments (MDS).
- Supervise Physical Therapy Assistants in accordance with state practice acts.
- Maintain treatment records, resident files, and progress notes; liaise with residents and families.
Qualifications & Skills:
- Current NY license and registration to practice as a Physical Therapist.
- Valid malpractice insurance and license in good standing.
- Dedicated to providing compassionate, high-quality care.
- Excellent communication, organization, and customer service skills.
- Ability to work independently and collaboratively in a team environment.
- Patience, cheerful disposition, and willingness to assist residents and staff.
- Flexible, able to work well under pressure.
- Covid-19 vaccination encouraged but not mandatory.
