We are seeking a Physical Therapist (PT) to provide high-quality rehabilitation services in a skilled nursing facility.
Key responsibilities include:
Planning, organizing, and implementing physical therapy programs in compliance with regulations Developing and evaluating treatment plans to promote residents’ physical, mental, and social well-being Participating in discharge planning and resident assessments (e.g., MDS) Supervising Physical Therapy Assistants per state practice acts Maintaining treatment records, resident files, and progress notes Communicating effectively with residents, families, and staff Qualifications and skills: Current license and registration as a PT in the state Malpractice insurance and good standing license Compassionate, energetic, and dedicated to quality care Excellent communication, organization, and customer service skills Ability to work independently and collaboratively, handle pressure, and adapt to team needs Patience, enthusiasm, and a genuine interest in resident well-being Note: COVID-19 vaccinations are encouraged but not mandatory.
