Role Overview:
Provides physical therapy evaluation and treatment for patients of all ages, adjusting care plans based on patient response and evidence. Serves as a clinical resource and role model for staff and students, ensuring safe patient transfers and promoting proper body mechanics.
Key Responsibilities:
- Designs individualized care plans incorporating goals, treatment, and discharge strategies.
- Educates patients, families, caregivers, and healthcare team members using effective teaching methods.
- Performs examinations, selects appropriate assessment tools, and documents findings per policy.
- Evaluates impairments related to orthopedic and neurologic conditions, completing objective testing.
- Applies therapeutic modalities and supports patient progress with communication to referring providers.
- Monitors support personnel and supervises students, contributing to departmental goals.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy.
- Licensed Physical Therapist (PT) in Illinois.
- Basic Life Support (BLS) certification.
- Strong clinical assessment, documentation, and communication skills.
- Knowledge of therapeutic modalities and patient safety protocols.
