Role: A Licensed Physical Therapist responsible for developing and implementing individualized treatment plans for patients of all ages in a community-focused setting.
Responsibilities:
- Evaluate patients’ physical conditions and establish treatment goals based on initial assessments.
- Develop and supervise personalized therapeutic exercise and treatment programs.
- Utilize modalities such as ultrasound, electrical stimulation, and manual techniques.
- Instruct patients and staff on exercises and movement principles for rehabilitation.
- Document progress, treatment outcomes, and communicate with referring providers.
- Supervise Physical Therapy Assistants, ensuring compliance with regulations.
- Participate in ongoing professional development and educational activities.
Qualifications & Skills:
- Licensed Physical Therapist (PT) with valid licensure in Illinois.
- Bachelor’s Degree in Physical Therapy.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment, instructional, and documentation skills.
- Ability to collaborate with multidisciplinary teams and support staff.
