Physical Therapist (PT) – Acute Care

Role Overview: Evaluates and treats patients of all ages, developing and modifying personalized care plans. Acts as a professional role model and clinical resource for staff and students, ensuring safe patient transfers, proper body mechanics, and respectful, empathetic care.


Qualifications: Requires a Bachelor’s Degree in Physical Therapy, licensure as a Physical Therapist (PT) in Illinois, and Basic Life Support (BLS) certification. Must complete ongoing education related to acute stroke patient care.


Key Responsibilities:

  • Designs and adjusts patient-centered treatment plans based on clinical findings and patient response.
  • Educates patients, families, caregivers, and healthcare team members using effective teaching methods.
  • Performs examinations, screens patients to determine therapy needs, and documents in accordance with policies.
  • Utilizes standardized outcome measures and collaborates with the care team on discharge planning and equipment needs.
  • Supervises support personnel, ensures quality care, and provides feedback.
  • Communicates patient progress, therapy recommendations, and discharge plans clearly to the medical team.
