Physical Therapist / PT – Live Well Homecare

We are seeking a dedicated Physical Therapist to provide home-based therapy in Virginia Beach and surrounding areas.

Key responsibilities include

assessing and treating patients with mobility, strength, and pain management needs

conducting assessments, documenting progress, and adjusting care plans accordingly

guiding patients and families with exercises, self-care, and safety education

collaborating with a home health team to meet individual care goals

The role requires a graduate of an accredited PT program, active Virginia PT license, current CPR certification, and up to 2 years of experience, preferably in home healthcare.

Ideal candidates possess

strong clinical judgment, interpersonal skills, dexterity, and compassion

a valid driver’s license and reliable transportation
