Job Summary:
Evaluate and treat patients primarily in Rehab and LTAC hospitals based on business needs. Responsibilities include daily documentation in EMR, patient and family education, communicating progress in clinical conferences, collaborating with team members, preparing equipment, and coordinating with vendors and outside physicians.
Responsibilities:
– Perform initial and discharge evaluations, set goals, and document treatment plans.
– Develop and deliver timely, effective therapy to meet functional goals.
– Identify educational needs and provide PRN.
– Participate as an interdisciplinary team member.
– Serve as a mentor for interns and participate in skills checks.
Minimum requirements include a Bachelor’s Degree in Physical Therapy (Doctoral preferred), at least 1 year of rehab experience, valid GA PT license or eligibility, BLS certification, and basic computer literacy.
