Role Overview: This position involves evaluating and treating patients primarily in rehab and LTAC hospitals, documenting care in the EMR, educating patients, families, and staff, and communicating patient progress during clinical conferences. The role includes coordinating with team members, managing equipment orders, liaising with vendors, and collaborating with outside physicians and professionals.
Key Responsibilities:
- Conduct initial and discharge evaluations, develop treatment goals, and document appropriately.
- Create and implement effective treatment plans to achieve functional goals.
- Assess educational needs and provide PRN therapy as required.
- Participate as a collaborative member of the interdisciplinary team.
- Serve as a clinical instructor/mentor for interns when needed.
- Engage in hospital and discipline-specific skills assessments.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
- Minimum one year of experience in rehab, SNF, LTACH, or acute care settings preferred.
- Valid BLS certification and GA PT licensure or eligibility.
- Basic computer literacy required.
