Role Overview: Provides outpatient physical therapy evaluation and treatment in a patient-centered, outpatient setting, working 1:1 with a diverse patient population. The role involves developing and implementing treatment plans, educating patients and healthcare teams, delegating tasks to assistants and students, and documenting care per departmental policies.
Responsibilities include:
- Establishing and adjusting treatment plans considering discharge needs and functional outcomes.
- Collaborating with patients, families, and healthcare providers, making referrals as needed.
- Delegating interventions to team members based on skill levels.
- Performing assessments, reassessments, and consultations.
- Ensuring accurate documentation of services provided.
Qualifications:
- Bachelor’s Degree in Physical Therapy (PT); Master’s Degree preferred.
- Valid Physical Therapist License.
- No prior experience required; supervision experience is not mandatory.
