Role Overview: The Physical Therapist provides primary rehabilitative care in patients’ homes within a designated multi-county area, focusing on improving their physical, emotional, developmental, psychosocial, and educational well-being.
Key Responsibilities:
- Assess, develop, and manage individualized care plans in collaboration with a multidisciplinary team, adjusting as needed to meet patient goals.
- Supervise and coordinate care provided by PTAs and Home Health Aides throughout the patient’s episode of care.
- Deliver age-appropriate therapeutic interventions and educate patients and caregivers to promote independence.
- Communicate patient progress and changes with physicians and team members, ensuring documentation aligns with regulatory standards.
- Participate in quality improvement initiatives, staff education, and ongoing professional development.
- Adhere to regulatory guidelines such as CMS, State, and Joint Commission standards, maintaining accurate documentation.
