Physical Therapist – Home Healthcare

Role Overview: The Physical Therapist provides primary rehabilitative care in patients’ homes within a designated multi-county area, focusing on improving their physical, emotional, developmental, psychosocial, and educational well-being.


Key Responsibilities:

  • Assess, develop, and manage individualized care plans in collaboration with a multidisciplinary team, adjusting as needed to meet patient goals.
  • Supervise and coordinate care provided by PTAs and Home Health Aides throughout the patient’s episode of care.
  • Deliver age-appropriate therapeutic interventions and educate patients and caregivers to promote independence.
  • Communicate patient progress and changes with physicians and team members, ensuring documentation aligns with regulatory standards.
  • Participate in quality improvement initiatives, staff education, and ongoing professional development.
  • Adhere to regulatory guidelines such as CMS, State, and Joint Commission standards, maintaining accurate documentation.

Qualifications & Skills:

  • Graduate of an accredited physical therapy program.
  • Current Physical Therapist license and Basic Life Support (BLS) certification.
  • Minimum of 1 year of full-time PT experience, preferably in home health.
  • Strong assessment, rehabilitation, organizational, and communication skills.
  • Proficiency with computer systems, home health software, and understanding of home health operations and regulations.

  • Additional Requirements:

    • Valid driver’s license and private vehicle insurance.
    • Ability to work independently, manage time effectively, and participate in call rotations as needed.
