Role: Physical Therapist, Home Care
Responsibilities:
- Conduct in-home assessments to evaluate patient needs.
- Develop and implement individualized treatment plans.
- Deliver therapeutic interventions to improve mobility, reduce pain, and enhance safety and independence.
- Collaborate with an interdisciplinary team to coordinate patient care.
- Support caregivers through education and guidance.
- Document patient outcomes and treatment progress using EMR systems.
Qualifications & Skills:
- Current licensure as a Physical Therapist.
- Strong clinical assessment and treatment skills.
