Role Overview
This position involves providing direct physical therapy care to home care patients in accordance with physician orders, focusing on assessment, treatment planning, and patient education.
Responsibilities include:
- Instructing patients and families on physical activities and assistive device use
- Re-evaluating patient progress and adjusting treatment plans
- Collaborating with physicians and other healthcare professionals to optimize care
- Fitting and modifying prosthetic and orthotic devices in coordination with specialists
Qualifications and Skills:
- Bachelor’s Degree in Physical Therapy (PT)
- Valid Physical Therapist License
- Valid Driver’s License
- No prior experience required
Key Duties:
- Providing patient-centered physical therapy services in the home environment
- Assessing patient needs and developing appropriate treatment strategies
- Educating patients and families on treatment procedures and device use
- Contributing to multidisciplinary team efforts to enhance patient recovery
