The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary team. Qualifications & Skills:

Educational requirements include a High School Diploma (N/A), or a Doctorate of Physical Therapy (required after 2015).
– Experience expectations involve a specified number of years and type of relevant experience.
– Licensure as a Licensed Physical Therapist is required, along with a valid Texas Driver’s License.
– Certifications include Basic Life Support (BLS), Auto Liability Insurance, and Reliable Transportation.
