Physical Therapist

Role Summary: This position involves evaluating and treating patients primarily in Rehab and LTAC hospitals, ensuring personalized care aligned with clinical goals. Responsibilities include conducting initial and discharge assessments, developing and implementing treatment plans, and documenting progress in the EMR. The role also encompasses educating patients, families, and staff, communicating patient status during clinical conferences, coordinating equipment and vendor contacts, and collaborating with outside physicians and team members.

Key Responsibilities:

  • Perform initial and discharge evaluations, set treatment goals, and document appropriately.
  • Develop and deliver effective therapy treatments to meet functional objectives.
  • Identify educational needs and provide PRN therapy.
  • Participate as an interdisciplinary team member and serve as a mentor for interns.
  • Engage in hospital and discipline-specific skills assessments.

Qualifications & Skills:

  • Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
  • GA Physical Therapy license or eligibility and BLS certification from the American Heart Association.
  • Minimum of 1 year experience in rehab settings such as SNF, LTACH, or acute care preferred.
  • Basic computer literacy is essential.
