Role Overview:
This position involves providing direct patient care activities to home care or home health patients based on physician orders. Responsibilities include assessing, evaluating, and planning treatments, instructing patients and families on treatment procedures, and supervising physical therapy practices.
Key duties encompass instructing patients in physical activities and device usage, re-evaluating and adapting treatment plans, collaborating with physicians and practitioners, and fitting or adjusting prosthetic and orthotic devices.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy (PT)
- Valid Driver’s License and Physical Therapist License
- No prior experience required
Additional Information:
- Full-time role with typical hours Monday through Friday, with holiday and weekend rotations
- No on-call responsibilities
- Community travel within Illinois with mileage reimbursement
