Role Overview:
We are seeking a Physical Therapist to provide patient-centered, skilled physical therapy services in a supportive rehab environment. The role involves evaluating patients, developing treatment plans, and delivering therapy to improve functional status and discharge readiness. Collaboration with patients, families, and healthcare teams is essential, along with delegating tasks to assistants and documenting care activities.
Key Responsibilities:
- Establish and implement treatment plans considering discharge needs and equipment.
- Educate and coordinate with patients, families, and healthcare providers, making referrals when needed.
- Delegate interventions appropriately, ensuring competency and safety.
- Complete accurate documentation in line with departmental policies.
- Conduct assessments, reassessments, and provide consultations as required.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy required; Master’s Degree preferred.
- Licensure as a Physical Therapist.
- Experience is not mandatory but preferred.
- Strong communication and collaborative skills.
