Role: Licensed Physical Therapist providing personalized, in-home and senior community care to older adults.
Responsibilities:
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
- Provide one-on-one care in private residences and senior living settings, including virtual care when appropriate.
- Document patient progress using electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place.
Qualifications & Skills:
- Valid PT license or eligibility in the practice state.
- Degree from accredited PT program, basic EMR and computer proficiency, current CPR certification.
- Ability to meet physical demands such as lifting, repositioning, and assisting patients, including driving to patient locations.
