Role Overview:
Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model. The position offers flexibility, autonomy, and the chance to build meaningful patient relationships, with options to incorporate telehealth and virtual care for enhanced access and continuity.
Key Responsibilities:
- Provide tailored physical therapy sessions in patients’ homes or communities
- Perform evaluations, develop individualized treatment plans, and administer evidence-based therapies
- Support patient mobility, independence, and wellness
- Document care accurately in electronic medical records
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging-in-place strategies
- Utilize telehealth when appropriate to extend care
Qualifications & Skills:
- Valid Physical Therapy licensure or eligibility
- Degree from an accredited PT program
- Proficiency in basic computer and EMR systems
- Current CPR certification
Physical Requirements: Ability to exert moderate effort, assist with patient transfers, perform repositioning, and drive to care locations, while wearing PPE as needed.
