Physical Therapist

Role Overview:
A Physical Therapist provides one-on-one geriatric care in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and evaluations.

Responsibilities:

  • Deliver personalized therapy sessions supporting patient wellness and functional independence
  • Develop and implement individualized care plans
  • Document care effectively within EMR systems
  • Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
  • Educate patients, caregivers, and staff on safety and aging in place
  • Incorporate telehealth and virtual care to extend access and flexibility

Qualifications & Skills:
  • Valid state Physical Therapy license or eligibility
  • Degree from an accredited program
  • Proficiency in computer and EMR systems
  • Current CPR certification

Physical & Other Requirements:
  • Ability to exert medium physical effort, lift up to 50 lbs, and assist with patient transfers
  • Repositioning, standing, walking, and climbing stairs
  • Driving to patient locations as part of care delivery
  • Wearing PPE when necessary
