Role Overview:
A Physical Therapist provides one-on-one geriatric care in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and evaluations.
Responsibilities:
- Deliver personalized therapy sessions supporting patient wellness and functional independence
- Develop and implement individualized care plans
- Document care effectively within EMR systems
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging in place
- Incorporate telehealth and virtual care to extend access and flexibility
Qualifications & Skills:
- Valid state Physical Therapy license or eligibility
- Degree from an accredited program
- Proficiency in computer and EMR systems
- Current CPR certification
Physical & Other Requirements:
- Ability to exert medium physical effort, lift up to 50 lbs, and assist with patient transfers
- Repositioning, standing, walking, and climbing stairs
- Driving to patient locations as part of care delivery
- Wearing PPE when necessary
