Role: Licensed Physical Therapist providing one-on-one patient care to older adults in private residences and senior living communities, supporting mobility, independence, and quality of life.
Responsibilities:
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions.
- Support patient wellness, mobility, and functional independence.
- Document clinical work accurately within EMR systems.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Utilize telehealth and virtual care to extend access and flexibility.
Qualifications & Skills:
- Valid PT license or eligibility in the practice state.
- Degree from an accredited Physical Therapy program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to meet physical demands of in-home care, including lifting, repositioning, and mobility tasks.
