Role Overview:
This position involves evaluating and treating patients primarily in Rehab and LTAC hospitals, with responsibilities including initial and discharge assessments, developing treatment plans, and documenting in the EMR. The role requires educating patients, families, and staff, communicating patient progress in clinical conferences, and collaborating with team members and external professionals. Additional duties include preparing equipment orders, contacting vendors, and serving as a mentor for interns. Participation in skills checks and interdisciplinary teamwork is essential.
Qualifications & Skills:
Applicants must hold a Bachelor’s Degree in Physical Therapy (Doctoral preferred), possess a valid GA Physical Therapy license or eligibility, and BLS certification. A minimum of 1 year of experience in rehab settings such as SNF, LTACH, or acute care is preferred. Basic computer literacy is required.
