Physical Therapist (Outpatient)
Seeking a compassionate, licensed Physical Therapist to deliver evidence-based outpatient care to diverse patient populations. Responsibilities include performing comprehensive evaluations, developing and progressing individualized treatment plans, and restoring function, mobility, and quality of life using manual therapy, therapeutic exercise, and appropriate modalities.
Key duties:
- Assess patients and identify impairments and functional limitations
- Create and update personalized treatment plans based on clinical findings
- Provide hands-on manual therapy, therapeutic exercise, and modalities
- Educate patients and families to support home programs and outcomes
- Collaborate with an interdisciplinary team to coordinate care
- Document clinical notes and progress in an EMR
- Contribute to clinical programs and ongoing professional development
Qualifications: Current PT licensure and strong clinical skills in evaluation and treatment planning, patient education, and teamwork in a patient-first environment.
