Primary Care Physician (Per Diem)
Provide modern, patient-centered primary care in a flexible, part-time role. Deliver comprehensive adult care through in-person clinic visits and 24/7 virtual care using a technology-enabled platform.
Key responsibilities
- Manage chronic conditions and coordinate ongoing care
- Deliver preventive care and health maintenance services
- Evaluate and treat same- or next-day acute needs as appropriate
- Collaborate with a multidisciplinary care team to support patient outcomes
- Use data and human-centered design principles to improve access, experience, and outcomes
Qualifications & skills
- Training and licensure to practice primary care medicine for adults
- Strong clinical judgment for preventive, chronic, and acute care
- Comfort delivering care via both in-person and virtual channels
- Collaborative mindset and commitment to continuous improvement
