Role: Provide in-office primary care and occasional telehealth services to patients of all ages for acute, chronic, and preventive needs. Manage patient health through comprehensive assessments, diagnosis, treatment planning, and ongoing care.
Key Responsibilities:
- Deliver primary care services in person and via telehealth.
- Address acute concerns, manage chronic conditions, and provide preventive care.
- Collaborate with in-office and virtual care teams to coordinate patient care.
- Perform procedures within the scope of clinical training.
- Provide patient education and support to promote adherence and health maintenance.
Qualifications & Skills:
- Clinical training and licensure to provide primary care.
- Strong diagnostic and clinical decision-making skills.
- Ability to work effectively in multidisciplinary, both in-person and virtual, care settings.
- Comfort performing procedures aligned with training and scope.
- Excellent communication and patient-centered approach.
