Patient Placement and Transfer Coordinator, RN – Night Shift

Role: Patient Placement and Transfer Coordinator RN

Responsibilities: Facilitate patient transfers within the hospital system, coordinating with physicians, staff, and departments to ensure timely and compliant placements. Manage daily calls for patient placement, ensuring adherence to protocols and regulations such as COBRA and EMTALA. Match patient needs with system resources to optimize care, provide follow-up communication, and report workflow trends for continuous improvement. Use problem-solving skills to determine best actions, delivering exceptional customer service and maintaining high standards of professionalism.

Qualifications & Skills: Graduate of an accredited nursing program, with a BSN preferred (or ADN with equivalent experience). Minimum of 3 years healthcare experience, including at least 2 years in acute hospital settings, preferably in Adult Critical Care or House Supervisor roles. Must hold a current RN license in Texas. Physical ability to lift, push, and pull weighted objects up to 50 lbs, with assistance. Strong communication, organizational, and problem-solving skills. Ability to provide high-quality customer service in a fast-paced environment.
