Role Overview:
Responsible for delivering complex, professional services to patients and their families within an interdisciplinary team setting.
Qualifications & Skills:
– Education: High School Diploma required; Master’s in Social Work preferred.
– Experience: At least 2 years in social work services, counseling, or related fields, with preferred experience in social work services and counseling.
– Certifications & Licenses: Must hold a valid license (LMSW or LCSW) and Basic Life Support (BLS); Level III SAFE Approach certification preferred.
– Key Skills: Strong interpersonal and communication skills, ability to work collaboratively, and familiarity with healthcare and social work protocols.
Key Duties:
- Provide interdisciplinary, patient-centered services ensuring quality care.
- Maintain necessary certifications and licensures, ensuring compliance with state requirements.
- Apply relevant experience in social work and counseling to support patient needs.
- Adhere to safety standards and protocols, including SAFE Approach Level III.
*Note: Certifications and licenses must be current in the State of Texas. BLS required within 90 days; Level III SAFE certification required within 6 months of hire.
