Palliative Care Nurse Practitioner (NP)

We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join our compassionate hospice team in Rochelle, IL, and surrounding areas.

Provide personalized palliative care by managing patient caseloads, including diagnosis, treatment, medication management, assessments, and consultations.

Travel to patients’ homes and facilities to deliver comprehensive end-of-life care with dignity, comfort, and peace.

Develop and enhance our palliative care program in collaboration with a multidisciplinary team.

Maintain current knowledge in hospice and palliative care practices.

Requirements include an active APRN and RN license, ability to obtain an NPI number, and a valid driver’s license. Preferred experience is 1-2 years in nursing, with a focus on compassion and integrity.
