Palliative Care Nurse Practitioner (NP)

We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join a compassionate hospice team serving Rochelle, IL, and surrounding areas.

Provide personalized, family-like care by managing a caseload of palliative patients in various settings, including homes and facilities.

Assist with patient diagnosis, treatment, medication management, assessments, and consultations, maintaining expertise in hospice and palliative care.

Travel to patient locations, ensuring compassionate and comprehensive end-of-life support.

Requirements include an APRN license, RN license, valid NPI (or ability to obtain), and a driver’s license. Preferred experience is 1-2 years in nursing, with a focus on compassion and integrity.
