Hospice RN (On-Call)
Provide compassionate, patient-centered hospice nursing support for individuals receiving care at home, including after-hours and weekend coverage.
Key Duties
- Triage incoming calls and assess changes in patient condition.
- Perform timely in-home visits to evaluate symptoms and manage pain and other needs.
- Coordinate care according to individualized hospice care plans.
- Collaborate with an interdisciplinary hospice team to support patient comfort and safety.
- Educate families on medications and end-of-life care, offering guidance and reassurance.
Required Qualifications & Skills
- Active RN licensure with hospice/home-care experience preferred.
- Strong clinical assessment skills and ability to make timely decisions.
- Excellent communication and compassionate bedside manner with patients and families.
- Sound judgment in managing pain and symptom control in a home setting.
- Ability to work effectively with an interdisciplinary care team.
